The form is approximately 9-1/4 inches by 12-1/4 inches. The form is printed in both German and Polish, and it was created for the purpose of registering members of the medical and health care professions. This form registered a female dental technician named Chana Lew, who was born in 1899 and who lived in Warsaw. Her religion is recorded on the form as “Mojżesz” (“Moses”), and the top of the form bears a stamp with the German term “Jude”, flanked on each side by a Star of David. The document shows age toning and a horizontal fold and it is in overall very good condition.
